I can’t say exactly what ZUN was thinking when he made Miko, but honestly I don’t think it even matters much? The most straightforward interpretation of her backstory (Prince Shotoku chose to have a woman’s body upon resurrection) is already simply the definition of a trans woman.

That said, ZUN seems to be an incredibly progressive person whenever he bothers to say anything on the subject. He’s openly scolded his co-hosts on a radio show for making homophobic jokes, he’s pro-immigration, pro-marijuana legalization, designed Okina with disability discrimination in mind, etc. He seems like a very opinionated, very political-minded person who simply keeps that out of his games because he doesn’t want to sound preachy.

I mean, Japan as a whole may be conservative, but ZUN is definitely not.
