Hitman NPCs are weird, the ones that he shoots in the start are kind of like "filler" crowd NPCs that die in one shot, if you accidentally kill a few of them in a place like the spectator stands a little farther in the map he's on, they'll start running out of the complex, but I think if you were to just pick off a few individual ones (especially with a silenced pistol) they might just freak out a bit but they don't activate the procedure to start running. In the Paris mission, for example, if you drop the light rig onto the stage, it'll kill enough of the bystanding crowd members to where they'll all start running out of the area, but if you, say, fire your gun once into the crowd, they'll just freak out and back away from the body while the guards scramble to find you. Since you're not really meant to go on huge rampages and actively play it like a shooter, they didn't put that much thought into reactions of civilians like individually running away.
